Job Description:

Schedule: Full Time

Shift: Monday - Friday 8:30am-5pm

Location: This is a fieldwork position covering Volusia and Flagler Counties.

• Provides personal care to patients according to the assignment details and care plan. Documents patient care and observations timely and appropriately. Participates in end-of-life care for patients and families.
• Informs the registered nurse of any changes in the patient's condition.
• Follows the schedule of care, including making several visits per day with travel.
• Assists patients with activities of daily living, such as bathing, dressing, and grooming.
• Monitors and records vital signs, including temperature, pulse, and respiration. Supports patients with mobility and exercise as prescribed.

Education:

• High School Grad or Equiv [Required]

Work Experience:

• Experience in the health care field [Required]

• Hospice experience [Preferred]

Additional Information:

• Hospice Aide training from an approved 75 hour training course [Required

Licenses and Certifications:

•Basic Life Support – CPR Cert (BLS)[Required]

• Driver's License (DL) [Required]

• Certified Nurse Assistant (CNA) [Required]

Pay Range:

$15.30 - $22.95
