• LPN : $35/hour
• RN : $45 /hour

What you'll need:

• Active LPN or RN License issued by NYS
• Minimum of 6 months experience in a home care setting.
• Current CPR/BLS Certification
• Empathy, Reliability , and a commitment to high-quality care

​What You'll do :

• Provide 1:1 nursing care and treatments according to the patient’s individualized care plan.
• Provide G-tube and PEG tube care, including feedings, medication administration, and site care.
• Perform tracheostomy care and suctioning, and manage ventilator support as ordered.
• Monitor the patient’s condition and promptly report changes to the appropriate healthcare professionals.
• Provide health promotion, counseling, and education to patients and caregivers.
• Administer medications and other prescribed interventions.
• Collaborate with healthcare professionals to coordinate patient care.
• Document patient assessments, care provided, and responses to treatment daily.
• Must be able to lift 50 lbs.
• Must be able to stand and work for extended periods.
• Perform additional responsibilities as required.
