Summary Job Description: BHMG Lexington - Vascular The Structural Heart Nurse Navigator is the point of contact for patients and families starting with the initial point of contact, telephone triage, and the screening process. This position follows the patient through the continuum of care including inpatient admission, 30-day, and one year follow-up. Ensures continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians, and daily patient management. The Structural Heart Nurse Navigator will collaborate with the interventional cardiologists, cardiothoracic surgeons and clinical staff of multiple departments to coordinate the care of patients with aortic stenosis who are being evaluated and/or selected for Transcatheter Tricuspid Valve Replacement (TCV) through their pre- and post-procedure care. This role will also coordinate the care of other structural heart disease patients, such as but not limited to MitraClip, TriClip. Also, participates in the development of marketing strategies, screening programs and other opportunities for program development. Minimum Requirements: Current Kentucky or Compact State unrestricted license as a Registered Nurse. Basic Life Support via the American Heart Association within 90 days of hire.
