Position Summary

Good Landing Recovery is seeking a compassionate and organized Mental Health Case Manager to join our team. The Mental Health Case Manager will work closely with clients experiencing mental health and co-occurring disorders, helping them access resources, develop treatment goals, and navigate the recovery process. This role requires strong interpersonal skills, attention to detail, and a passion for supporting individuals on their journey toward stability and wellness.

Key Responsibilities

  • Conduct client assessments and assist in developing individualized service plans.
  • Coordinate care and collaborate with therapists, medical providers, community agencies, and family members when appropriate.
  • Monitor client progress and provide ongoing case management support.
  • Assist clients with accessing housing, employment, healthcare, transportation, and other community resources.
  • Maintain accurate and timely documentation in accordance with agency, state, and federal requirements.
  • Advocate for clients to ensure they receive appropriate services and support.
  • Participate in treatment team meetings and case reviews.
  • Provide crisis intervention and support as needed.
  • Educate clients and families about mental health resources and recovery services.
  • Ensure compliance with confidentiality and ethical standards.

Qualifications

Required:

  • Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field.
  • Minimum of 1 year of experience working with individuals experiencing mental health and/or substance use disorders.
  • Strong communication, organizational, and problem-solving skills.
  • Knowledge of community resources and mental health service systems.
  • Ability to maintain professional boundaries and work effectively with diverse populations.
  • Proficiency in electronic health records (EHR) and Microsoft Office applications.

Preferred:

  • Case management certification.
  • Experience in behavioral health, addiction treatment, or recovery services.
  • Bilingual abilities are a plus.
