Position Purpose
In the Emergency Department (ED) at Saint Francis Hospital and Medical Center, the Nurse Senior – Navigator, informally known as the Complex Care Coordinator, plays a crucial role in supporting emergent and high-risk patients with complex medical, social, and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated, patient-centered care and support safe, effective transitions across the continuum.
What You Will Do
- Conduct comprehensive, real-time assessments of patients’ medical, psychosocial, and behavioral health needs upon presentation to the Emergency Department.
- Collaborate closely with Emergency Department physicians, advanced practice providers, nursing staff, case management, and ancillary teams to support individualized care planning for complex and high-risk patients.
- Coordinate referrals and facilitate connections to appropriate inpatient services, specialty providers, community resources, and support programs to promote safe dispositions and effective transitions of care.
Minimum Qualifications
Required:
- Graduation from an accredited nursing program, with a Bachelor of Science in Nursing (BSN) strongly preferred and generally expected for successful candidates.
- Valid RN license in Connecticut.
- The ideal candidate will have a minimum of three (3) years of Emergency Department and/or acute inpatient nursing experience, with demonstrated ability to manage complex, high-risk patient populations.
Preferred:
- Emergency Department Case Management or care coordination experience strongly preferred.
- BLS/CPR certification (American Heart Association or American Red Cross).
Position Highlights

- Compensation: Salary range $86,304 – $133,328, based on a 40-hour full-time equivalent; compensation will be prorated based on a 24-hour workweek
