The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes according to physician orders and individualized plans of care. The OT evaluates patients, develops and implements treatment plans, promotes independent functioning, provides patient and caregiver education, coordinates care with interdisciplinary team members, and documents services in compliance with agency policies and regulatory requirements. The OT also supervises Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs) as appropriate.
This role is ideal for an occupational therapist who is passionate about helping patients maximize their independence and quality of life in the home setting.
