Position Purpose
Mercy Care Atlanta is seeking a compassionate and dedicated Peer Support Specialist to join our mission-driven team. In this role, you will use your lived experience, recovery journey, and professional skills to support individuals who are currently experiencing or have previously experienced homelessness. You will help foster hope, connection, wellness, self-advocacy, and recovery while empowering individuals to achieve their personal goals and improve their overall quality of life. Working closely with multidisciplinary care teams, you will play a key role in ensuring continuity of care and promoting long-term stability and well-being.
What You Will Do
As a Peer Support Specialist, you will:
- Build trusting relationships with individuals and provide peer-based support and encouragement.
- Promote recovery, wellness, self-direction, socialization, and community integration.
- Assist clients in developing skills to manage symptoms, navigate challenges, and access community resources.
- Support individuals in identifying strengths and establishing meaningful recovery and life goals.
- Facilitate activities that help clients communicate health concerns, track progress, and advocate for their needs.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans.
- Serve as a client advocate and provide expertise informed by lived experience.
- Monitor progress through regular follow-up and assist with revising goals and care plans as needed.
- Maintain accurate and timely documentation, case notes, and records in compliance with organizational and regulatory standards.
- Ensure compliance with HIPAA and confidentiality requirements.
- Participate in supervision, team meetings, continuing education, and professional development opportunities.
- Perform other duties as assigned to support the mission of Mercy Care Atlanta.
Minimum Qualifications
- Georgia Certified Peer Specialist certification or eligibility to obtain certification within six (6) months of hire.
- Strong understanding of recovery-oriented care and psychosocial rehabilitation principles.
- CPR and First Aid certification.
- Valid Georgia driver's license with the legal ability to drive in Georgia.
- Reliable transportation to and from the worksite.
- Minimum of two (2) years of peer support experience.
- Experience working with individuals experiencing homelessness with complex social and healthcare needs preferred.
