Job Description Summary:

This is a unique role requiring critical care or emergency department experience (3-5 years minimum). Prioritization and ability to triage/assimilate patient information quickly is essential in our fast past, high volume department. Computer skills and basic computer troubleshooting is required to be successful. Capacity to communicate clearly and manage stressful situations involving critically ill patients with life threatening emergencies is also essential for every team member.

Transfer Center Nurse role: 24/7 department that requires weekends and holiday rotation. TCRN triages calls from sending providers/facilities that desire to send patient to an OhioHealth (OH) facility often due to limited resources available from sending facility. Obtain essential report items, contacting appropriate receiving OH hospital, and accepting/consulting providers/physicians utilizing resources and protocols. Communication skills are vital to ensure patients are receiving quality efficient care. Requires multi-tasking several cases at a time while providing quick acceptance, timely follow-up with sending and receiving teams, and setting up appropriate transportation (for OH to OH facilities) based on urgency of transfer. Organization skills and ability to focus is required to follow similar but varying processes for each of our facilities and the service lines at each hospital. The Transfer Center team also manages the bedding of patients in several OH hospitals and is a role that is added to the associate’s responsibilities after an appropriate foundation is established as a Transfer Center RN. Types of cases frequently managed by team: Stroke Alerts, STEMI Alerts, Trauma Alerts, Cardiac Arrests, Critical limbs, OB/GYN emergencies, etc.

Responsibilities And Duties:

20% Maintains flow of patients throughout the hospital to include: emergency department, operating rooms, invasive recovery, cath labs, and nursing units.

20% Assesses Hospital admission to ensure appropriate admissions, level of care, medical necessity and utilization of resources. 1. Telephones triage and reviews documentation to assure outlying hospital direct admissions/ED transfers receive appropriate and timely bed management 2. Initiates clinical pathways on emergency and direct admissions 3. Screens observation admissions for appropriateness with approved criteria and directs to most optimal level of care. 4. Contacts physicians regarding order clarification and change in appropriateness for level of care needs. 6. Activates patients to assigned unit and bed. Assures correct assignment of status.

18% Plans and coordinates safe transfer of inpatient patients to accommodate needs of each admission. Works with nursing units to identify patients to transfer to lower levels of care to accommodate patients requiring higher acuity.

15% Collaborates with nursing administration to assure all resources are allocated to best meet the needs of organization.

9% Evaluates each patient continuously to identify change in required level of care to assure appropriate placement GMB versus Tele or telemetry versus intermediate or critical care versus telemetry, workup issues and admissions versus discharges.

5% Provides superior customer service to meet needs of patients, families, physicians, outlying hospitals, physician offices, nursing facilities, urgent cares, home care, hospice, and other telephone contacts received by department.

4% Provides education and support for bed management practice to interns, residents, and attending physicians.

5% Provides resources for organization regarding a variety of topics including: policy/procedure, department location, crisis management, staffing, department responsibilities and various other topics.

2% Identifies opportunities for process improvement and provides ongoing feedback.

2% Acts as liaison between all central Ohio hospitals to identify bed availability within Central Ohio area in order to assure timely and accurate placement of patients.

As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties.

Additional Job Description:

Effective verbal and written communication, problem solving and organizational skills; computer skills (CareConnect); typing skills, Clinical pathway development and understanding of quality improvement. 3 - 5 yrs. in emergency department/critical care; previous leadership roles (charge, preceptor).
