Job Summary: The Nurse Navigator is a key member of the Joint Replacement and Spine Center program, providing for time-sensitive orthopedic nursing expertise followed by continued clinical oversight and care coordination for patients admitted with a diagnosis of orthopedic and spine within the hospital. This role assists in the execution and evaluation of evidence-based processes to achieve the goal of excellent care and expected outcomes.

Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License.

Work Experience: Minimum 5 years in Nursing related field and Minimum 2 years of Orthopedic experience.

Knowledge, Skills, and Abilities: Knowledge of Microsoft 365 and other applicable software. Excellent communication skills, both written and verbal that present clear and concise information. Proficient in applying nursing theories, evidence-based nursing knowledge, and problem-solving skills. Knowledge of pathophysiology of specific disease processes and orthopedic treatment modalities and evidence-based treatment pathways, including preoperative evaluation and optimization, surgical indications, joint replacement and spine protocols, Venous thromboembolism/Deep Vein Thrombosis prevention, infection prevention, pain management, early mobilization and rehabilitation, discharge planning and follow-up, postoperative care coordination and management and recognition of signs and symptoms of orthopedic decline. Understanding of current clinical practice guidelines and standards for orthopedic treatment plans and care coordination across the continuum. Effective skills in collaboration, coaching, mentoring, listening, participative decision-making, negotiating and team building. Ability to plan, develop, implement and administer health related programs. Ability to work autonomously, establishing benchmarks for success, and reporting these. Ability and desire to establish and maintain rapport with physicians and organizational personnel.

Essential Functions and Responsibilities: Provides clinical oversight and coordinates multidisciplinary care of patients with an orthopedic diagnosis and/or procedures according to orthopedic center-of-excellence standards, evidenced-based practices and established clinical guidelines. Establishes and maintains patient and family relationships by assessing understanding of the diagnosis, planned procedure, treatment plan, medications, recovery expectations, and healthcare resources. Assesses and addresses patient education and resource needs for Joint Replacement and Spine Center program patients, ensuring appropriate education, materials and support are provided. Collaborate with clinical staff, managers, case management, therapy, physicians and other interdisciplinary team members to address patient needs and barriers to care. Performs nursing practices proficiently through accurate and timely documentation, appropriate referral process, clinical assessment, patient education, and effective communication with member of healthcare team. Oversees continuity of care and remains a point of contact throughout the patient's hospitalization. Coordinates program support for joint replacement and spine patients through education and caregiver support discussions. Actively collaborates with the clinical staff and the patient/representative during the hospitalization and remains available for post-discharge questions, concerns, and care coordination. Participates in joint replacement and spine center support, including staff training. Actively participates in Joint Replacement and Spine Center program activities, including staff education and training, committees, process improvement initiatives, and program workgroups. Contribute to efforts that promote quality, patient safety, patient experience, timely care, and cost-effective delivery of services. Assists with the review of patient and program data, including quality, clinical, operational, and patient experience outcomes, to identify trends, opportunities for improvement, and areas requiring follow-up. Support data-driven decision-making and performance improvement initiatives. Coordinates and support the collection completion and review of Patient-Reported-Outcome Measures (PROMs) for Joint Replacement and Spine Center patients. Monitors PROM participation and outcomes and collaborates with interdisciplinary team to support data-driven quality improvement and program outcomes. Represents joint replacement and spine center program/orthopedic service line in collaboration with physician and organizational partners to support referral relationships, coordinated outreach and educational initiatives, program’s development and ongoing program operations.

Decision Making: The carrying out of non-routine procedures under constantly changing conditions, in conformance with general instructions from a supervisor.

Working Relationships: Coordinates activities of others (does not supervise). Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
