Compensation Range:

$21.40 - $28.89

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

Ensure timely follow-up on requests to help guide patients through all the various complexities of the healthcare system. By running and reviewing non-adherence reports, actively engage patients and build strong relationships with patients to help them stay engaged in medical care and improve adherence to recommended care goals.

The Patient Navigation Team is committed to removing patient barriers to care by identifying critical resources, helping them navigate through health care services and systems, and promoting health. They work closely with the Care Team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes. They are responsible for active patient outreach, scheduling services, assisting with insurance coverage, as well as addressing any inquiries or complaints.

ESSENTIAL FUNCTIONS Responsible for comprehensively supporting coordination of patient care:

• Actively engage patients in need of services and/or have been reported as non-adherent to recommended care goals.
• Respond timely to eRecord and e-mail requests from other clinical team members.
• Provide superior customer service and establish close relationships with patients to build trust within the health care system.
• Communicate with Care Team members (Care Coordinators, primary care physicians and other health care providers) to facilitate patient care.
• Identify and coordinate resources for patients to overcome barriers to care, such as transportation or preferred clinic visit times.
• Provide health-related education and promotion, and support.
• Provide support and information for patients needing assistance obtaining insurance through the NYS Health Exchange.
• Maintain eRecord documentation of all patient encounters and complete reporting requirements according to organization standards.
• Adhere to Patient Navigator Workflow Guidance - run required reports as instructed, to identify patients that are non-adherent with prescribed treatment/care goals and perform necessary outreach to engage patients.
• Participate in clinical and staff team meetings to understand clinical priorities and make recommendations as needed for process improvement.

MINIMUM EDUCATION & EXPERIENCE

• Bachelor's degree in healthcare related field required
• Or equivalent combination of education and experience
• 1-3 year experience in community health, human services, and/or patient liaison/advocate roles preferred

LICENSES AND CERTIFICATIONS

• BCPA-Board Certified Patient Advocate upon hire preferred and
• DL NUMBER - Driver License, Valid and in State plus a satisfactory driving record and access to reliable transportation upon hire required
