Key/Essential Functions & Responsibilities

  • Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
  • Facilitate and lead Intake/admission meeting.
  • Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans
  • Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director
  • Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan
  • Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.
  • Performs all other duties, as assigned.

Pay Rate: $87,560.30 annually

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)

  • A Master’s Degree in Psychology, Social Work, Counseling and five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.
  • Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.
  • Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.

Preferred Qualification Requirements (desired requirements beyond MQRs above)

  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential
