Осмотр пациента. Сбор анамнеза с выявлением кардиологических рисков. Физикальное обследование, включая прослушивание сердечных тонов, измерение а/д и пульса, оценку наполненности артерий и вен, определение лодыжечно-плечевого индекса и выявление отеков. Диагностика. Направление на инструментальные (УЗИ, КТ/МРТ сердца, радионуклидные тесты) и лабораторные исследования. Врач-кардиолог делает ЭКГ, электрокардиографию и пробы с нагрузкой, расшифровывает полученные результаты. Доктор ставит диагноз в соответствии с МКБ, при необходимости направляет на консультацию к другим специалистам. Анализ рисков. Оценка тяжести состояния больного и вероятности развития угрожающих жизни осложнений. Определение целесообразности стационарного лечения, хирургического вмешательства или вживления кардиостимулятора. Лечебные мероприятия. В обязанности врача-кардиолога входит планирование комплексного лечения по действующим протоколам. Специалист подбирает лекарственные препараты, оценивая эффективность дозы, скорость введения инфузионных средств и взаимодействие разных групп медикаментов. Доктор назначает немедикаментозную терапию: лечебное питание, физическую нагрузку, физиопроцедуры. Контроль эффективности. Кардиолог выписывает направления на повторные исследования и оценивает клинические критерии улучшения состояния. При необходимости проводится коррекция лечебного курса. Диспансеризация. Включает мониторинг состояния кардиопациентов на повторных осмотрах. Врач принимает решение о направлении больного на экспертизу по стойкой нетрудоспособности. Важно раннее выявление сердечно-сосудистой патологии при диспансерном наблюдении сотрудников предприятий. Профилактика. Предотвращение развития тяжелых осложнений, включая нежелательную реакцию на лекарственные препараты. Врач-кардиолог беседует с пациентами о пользе здорового образа жизни, проводит лекции для медперсонала о соблюдении санитарных норм. Другая медицинская помощь. При критическом состоянии пациента врач-кардиолог обязан оказать медпомощь для стабилизации жизненных показателей. Он должен иметь опыт проведения сердечно-легочной реанимации. Заполнение документации. Кардиолог выписывает рецепты и больничные листы, заполняет учетно-отчетные журналы. Доктор анализирует статистические данные работы кардиослужбы и предпринимает действия для ее оптимизации. Работа с подчиненными. Доктор контролирует действия младшего и среднего медперсонала, своевременно устраняет недочеты в их деятельности. Поддержание принципов деонтологии. Врач использует психологические приемы общения с пациентами и их родственниками. Медработник заботится о собственном психологическом комфорте, минимизируя риски профессионального выгорания. В обязанности врача-кардиолога входит неукоснительное соблюдение врачебной тайны.
