The Role:
The Community Supports Coordinator supports the operational functions of the Agency with Choice service line within Community Supports. This role manages the onboarding of new members and caregivers, helping move referrals through admission and ensuring required documentation, compliance, and service requirements are completed accurately and efficiently.
The Community Supports Coordinator serves as a primary point of contact for members and caregivers throughout onboarding, providing timely communication, resolving questions, and partnering with internal teams, Case Managers, and health plans to support a smooth transition into services.
Primary Responsibilities
- Manage the AWC onboarding process from referral through admission, coordinating both member admission requirements and caregiver hiring activities.
- Guide caregivers through onboarding, including application follow-up, compliance requirements, documentation, orientation, system training, and timekeeping expectations.
- Support new members throughout admission, ensuring required documentation is completed and providing timely updates regarding caregiver onboarding and the initiation of services.
- Respond to member and caregiver inquiries by phone, text, chat, and email, providing timely resolutions and coordinating with internal or external partners when additional support is needed.
- Maintain accurate member and caregiver information in Salesforce and other tracking systems while supporting health plan, Case Manager, closed-loop referral, care integration, and HIPAA requirements.
This is a hybrid position, coming into the El Segundo office 1x per month.

What You Bring to the Table:
Qualifications
- High school diploma or GED required; Bachelor’s degree preferred.
- Minimum 1 year of customer service or other service-oriented experience.
- Experience supporting customers through an onboarding, enrollment, or similar multi-step process.
- Experience working independently while managing multiple priorities and time-sensitive deadlines.
- Spanish language skills preferred.

The expected California Pay Range for this position:
$23—$24.84 USD
