Pour l'un de nos clients situé dans la région Riviera\-Chablais, nous recherchons un·e

Éducateur·trice Social·e à 80% \- Riviera Chablais

afin de rejoindre une structure médico\-sociale spécialisée dans l'accompagnement de personnes âgées présentant des besoins spécifiques en santé mentale et en psychiatrie de l'âge avancé.

Votre mission

Au cœur d'une équipe pluridisciplinaire engagée, vous accompagnez les résident·e·s dans leur parcours de vie en valorisant leurs ressources, leur autonomie et leur bien\-être. Vous contribuez activement au maintien du lien social et à la mise en œuvre de projets individualisés adaptés à chaque situation.

Vos principales responsabilités

  • Concevoir et mettre en œuvre des projets socio\-éducatifs favorisant l'expression, l'autonomie et le maintien des capacités des résident·e·s
  • Observer, analyser et adapter les accompagnements en fonction de l'évolution des besoins
  • Participer à l'élaboration, au suivi et à l'évaluation des projets thérapeutiques individualisés
  • Développer une relation de confiance avec les résident·e·s et leur entourage
  • Collaborer étroitement avec les différents professionnels impliqués dans l'accompagnement
  • Favoriser une dynamique de vie stimulante et porteuse de sens au quotidien

Votre profil

  • Diplôme d'Éducateur·trice Social·e ou titre reconnu par le SEFRI, obligatoire
  • Expérience dans le domaine médico\-social, idéalement auprès de personnes âgées
  • Intérêt marqué pour l'accompagnement en santé mentale et en psychiatrie de l'âge avancé
  • Connaissances en addictologie et/ou formation complémentaire (CAS), un atout
  • Capacité à élaborer et conduire des projets socio\-éducatifs individualisés
  • Aisance dans le travail en réseau et en interdisciplinarité
  • Excellente maîtrise du français, à l'oral comme à l'écrit
  • Bonne maîtrise des outils informatiques usuels
  • Connaissances de l'approche systémique, un avantage
