Sobre el cliente:

Estamos buscando a un/a Médico Especialista en Medicina Familiar y Comunitaria para integrarse en un Área Básica de Salud (ABS) de referencia en la provincia de Girona.

Buscamos a un/a profesional con clara vocación asistencial, orientado/a a la atención integral del paciente, la promoción de estilos de vida saludables, el control de cronicidades y el trabajo en el ámbito domiciliario y comunitario.

Descripción del puesto:

En dependencia de la Dirección del Área Básica de Salud (ABS), el/la profesional asumirá las siguientes responsabilidades:

  • Atención Médica Integral y Continuada: Consultas presenciales, telefónicas y atención domiciliaria para asistencia programada y demanda espontánea/urgente.
  • Promoción de la Salud y Prevención: Actividades de educación sanitaria individual y comunitaria (colegios, institutos e instituciones locales), campañas de vacunación y atención prioritaria a colectivos vulnerables y grupos de riesgo.
  • Manejo del Paciente Crónico: Diagnóstico precoz, control y seguimiento continuado de patologías crónicas (cardiopatías, diabetes, hipertensión, patologías respiratorias y salud mental no compleja), optimizando la revisión del tratamiento para evitar la polimedicación.
  • Gestión Clínica y Documental: Uso del software asistencial corporativo (E-CAP / HES), emisión de recetas electrónicas, gestión de Incapacidades Temporales (bajas/altas IT), certificados médicos e interpretación de pruebas complementarias (ECG, espirometrías, analíticas, radiografías, etc.).
  • Coordinación Multidisciplinar: Trabajo estrecho con enfermería, trabajo social, especialistas de la red hospitalaria, salud mental (ASSIR/PSP) y centros sociosanitarios.
  • Calidad y Docencia: Participación en reuniones clínicas, comisiones de calidad, revisión de indicadores asistenciales (EQA / IQPF) y formación continuada/docencia a residentes o estudiantes.

Requisitos Imprescindibles:

  • Licenciatura o Grado en Medicina (homologado si procede).
  • Título de Especialista vía MIR (o homologación oficial) en Medicina Familiar y Comunitaria.
  • Colegiación activa en un Colegio Oficial de Médicos en España.
  • Capacidad de organización de consulta y destreza en informática médica asistencial.

Requisitos Valorables:

  • Experiencia previa en Equipos de Atención Primaria (EAP) o centros de salud.
  • Formación de posgrado o máster en Atención Primaria o Atención Pre-hospitalaria.
  • Conocimiento del modelo de salud de Cataluña (red SISCAT) y del sistema informático E-CAP / HES.
  • Conocimientos de catalán o disposición para su aprendizaje.
