Palliative Care APP-AdventHealth at Porter

Annual Compensation $133,435 - $142,821

AdventHealth is an established nationwide nonprofit connected system of care, with a dedicated Primary Care division, consistently leading in quality, safety, and patient satisfaction. With the mission of Extending the Healing Ministry of Christ, AdventHealth focuses on providing whole person care to heal the mind, body, and spirit. We are looking for a physician who will model and lead excellence in clinical practice, doctor-patient relationships, and professionalism while maintaining AdventHealth's Mission, Vision, Values. AdventHealth Medical Group (AHMG} is a sister organization of AdventHealth which employs 400+ physicians in 25+ individual service lines in the Rocky Mountain Region. This aligned model and practice management experience provides the opportunity for physicians to focus on the clinical aspects of medicine. This is an employment opportunity that includes the benefits of call coverage and competitive compensation with a productivity model.

Job Description:

Responsibilities:

  • Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
  • Performs patient rounding with an interdisciplinary team and participates in case conferences.
  • Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.
  • Evaluates patient's response and intervenes to ensure optimal patient outcomes

QUALIFICATIONS:

  • Palliative care experience desired - inpatient or outpatient
  • Bachelor's degree, Nursing- Required
  • Master's degree, Nursing- Required
  • Basic Life Support-American Heart Association - Required
  • Registered as Advanced Practice Nurse - Required
  • Registered on the State of Colorado Advanced Practice Registry - Required
