Job Description:
- Facilitates timely scheduling with coordinators, supervisors, physicians, and patients.
- Monitors high-risk cardiovascular patients and provides necessary support.
- Participates in performance improvement activities to enhance cardiovascular service delivery.
- Maintains professional relationships with all customers and stakeholders.
- Acts as a liaison to physicians’ offices regarding proper orders, medical records, consents, and other required documentation.
- Coordinates multidisciplinary cardiovascular services including cardiology, cardiothoracic surgery, and vascular surgery.
- Develops and implements evidence-based measures to improve the quality of cardiovascular care.
- Ensures compliance with all regulatory guidelines related to cardiovascular services.
- Supports and assists with data collection for process and quality improvement initiatives.
- Manages patient access and scheduling within the health system for cardiovascular services.
- Provides patient education and serves as a resource for patients and families during diagnosis and treatment.
- Communicates patient status and needs to healthcare teams and collaborates with other providers to sequence care.

- Knowledge, Skills, and Abilities:

- Knowledge of PACS, Heartlab, LumeDx, Cerner and associated systems [Required]
- Previous leadership experience [Preferred]
- Previous experience with projects or program start-up [Preferred] Computer skills such as Microsoft Word, Access, Excel and Outlook [Preferred]
- Education:
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Required]
- Master's [Preferred]
- Work Experience:
- 2+ years acute care experience [Preferred]
- 2+ years in cardiac care [Preferred]
- Licenses and Certifications:
- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:
$33.08 - $58.04
