The Wellness Nurse supports Director of Resident Services, to manage the day-to-day healthcare operations of the Independent Living community to ensure resident’s healthcare needs are met. This is an every other weekend (Saturday and Sunday) position from 7:00am-3:30pm.

Responsible for providing emergency intervention to residents to facilitate the residents’ well-being and providing nursing intervention to facilitate the individual resident's independence. Ensures residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change. Supervises the maintenance of resident charts. Facilitates continuity of care for those residents receiving home health care, hospice services, and other third-party healthcare-related services.

Education and Experience

LPN Graduate of an accredited school of professional nursing and a minimum of 5 or more years of acute hospital nursing or home health experience. Possesses the ability to make independent decisions when circumstances warrant such action. Demonstrates an interest in working with a senior population. Ability to establish an effective working relationship with all departments and the community. Excellent organizational and follow-through skills. Demonstrates the ability to make appropriate judgments based on sound knowledge of nursing principles, procedures and elements of patient teaching.
