Die Alterszentrum Breitlen AG bündelt die stationäre und ambulante Grundversorgung in der Gemeinde Hombrechtikon. Zum Alterszentrum Breitlen gehören ein modernes Alters- und Pflegeheim mit 61 Zimmern, die Spitex Hombrechtikon, über 50 Seniorenwohnungen sowie die Beratungsstelle «Alter und Gesundheit». Rund 130 Mitarbeitende arbeiten für das AZ Breitlen, wie wir uns selber nennen.

Für die Spitex Hombrechtikon suchen wir per sofort oder nach Vereinbarung eine engagierte und flexible

Deine Aufgaben umfassen

  • Effiziente Organisation und Koordination des täglichen Arbeitsablaufes für die Teams der Pflege und der Hauswirtschaft - auch Tourenplanung genannt.
  • Anlaufstelle für sämtliche Anliegen, die mit der Disposition zusammenhängen
  • Planung von hauswirtschaftlichen und pflegerischen Dienstleistungen
  • Fachliche Beratung von internen und externen Stellen und Zuweisern bzgl. Spitexleistungen
  • Telefonische Vorabklärungen
  • Hauswirtschaftliche Bedarfsabklärungen mit dem Instrument Inter RAI HC
  • Mitarbeit im administrativen Bereich

Das bringst du mit

  • Eine medizinische Grundausbildung wie MPA, FAGE, Fachfrau/-mann Apotheke EFZ
  • Von Vorteil Spitexerfahrung, ist aber nicht zwingend
  • PC-Kenntnisse
  • Schnelle Auffassungsgabe
  • Durchsetzungsvermögen
  • Freude am Organisieren und Planen
  • Interesse, Prozesse mitzugestalten
  • Du schätzt den Klientenkontakt persönlich, telefonisch oder per Mail und bist kundenorientiert.

Wichtig: Dass du am Freitag arbeiten kannst, ist Voraussetzung. Der zweite Tag ist verhandelbar.
