PRINCIPAIS RESPONSABILIDADES

  1. Realizar o acompanhamento farmacoterapêutico dos pacientes em atendimento domiciliar, promovendo o uso seguro, racional e eficaz dos medicamentos.
  2. Avaliar prescrições médicas, identificar potenciais interações medicamentosas, incompatibilidades e oportunidades de otimização da terapia farmacológica.
  3. Orientar pacientes, familiares e equipes assistenciais quanto ao armazenamento, preparo, administração e monitoramento dos medicamentos.
  4. Monitorar indicadores clínicos e farmacêuticos, registrando intervenções e contribuindo para a segurança do paciente e a qualidade da assistência.
  5. Atuar de forma integrada com a equipe multiprofissional, participando da elaboração e acompanhamento dos planos de cuidado domiciliar.

É NECESSÁRIO QUE VOCÊ TENHA

  1. Ensino superior completo em Farmácia.
  2. Registro ativo no CRF.
  3. Experiência em Farmácia Clínica, Assistência Farmacêutica ou Home Care.
  4. Conhecimento em farmacoterapia, validação de prescrições e segurança do paciente.

DIFERENCIAIS

  1. Pós-graduação em Farmácia Clínica, Farmácia Hospitalar ou áreas correlatas.
  2. Experiência em Atenção Domiciliar (Home Care).
  3. Conhecimento no sistema Tasy.
