Andante is één van de 17 Centra voor Geestelijke Gezondheidszorg in Vlaanderen. We zijn een dynamische organisatie gespecialiseerd in preventie, vroegdetectie en ambulante behandeling van ernstige psychische problemen. Vanuit onze maatschappelijke betrokkenheid werken we nauw samen met partners binnen en buiten de geestelijke gezondheidszorg.

Andante is een van de partners van het netwerk PANGG 0-18 en neemt in deze samenwerking het werkgeverschap voor het mobiel crisisteam op zich.

Want wij zijn op zoek naar een enthousiaste en flexibele collega voor het mobiele crisisteam voor kinderen en jongeren.

Wat zijn je taken?

  • je gaat op huisbezoek bij kinderen, jongeren en gezinnen die in een crisissituatie verkeren, waarbij er een vermoeden is van een psychische of psychiatrische problematiek (o.m. suïcide); je onderzoekt de psychosociale situatie en context van cliënten (sociale diagnostiek) op alle levensdomeinen (inkomen en materiële hulp, werk, school, wonen, zinvolle (vrije)tijdsbesteding, relaties in het gezin, opvoeding en kinderen, sociale contacten, gezondheid, juridische zaken, ...); je ondersteunt cliënten en hun context bij concrete hulpvragen die zij hebben op deze domeinen en bij het benutten van hun rechten in de samenleving; je verruimt/versterkt of creëert een ondersteunend netwerk wanneer dit afwezig is; je leidt cliënten warm toe naar relevante andere diensten; je legt contacten met andere diensten die betrokken zijn op de cliënt en organiseert hiervoor indien nodig rondetafels met de betrokken partijen (cliënt, context, andere hulpverleners); je werkt oplossings-, kracht- en actiegericht, met aandacht voor het netwerk van het gezin; je werkt samen met andere partners van het crisisnetwerk en aanmelders zoals daar zijn ELP, CLB, huisartsen, e.a. je bent aanspreekpunt voor en geeft advies aan collega’s wanneer in lopende begeleidingen praktische vragen opduiken; je hebt zin om de rol van maatschappelijk werker in een mobiel crisis team mee vorm te geven.

  • heb je een goede kennis van psychische en psychiatrische problematieken bij kinderen en jongeren, hun aanpak en begeleiding;
  • kan je zelfstandig werken en neem je initiatief binnen een multidisciplinaire teamcontext;
  • heb je een bachelordiploma in een sociale richting;
  • heb je een rijbewijs B en een eigen wagen die je kan gebruiken voor je mobiele activiteiten.
