Om ASIH Tyresö/Söderort

ASIH Tyresö/Söderort är en liten vårdgivare som bedriver avancerad sjukvård i hemmet på uppdrag åt Region Stockholm sedan 2015. Vi arbetar i område tre med upptagningsområde Tyresö, Haninge, Huddinge, Botkyrka och Nynäshamn Vi arbetar i ett litet multiprofessionellt team bestående av Läkare, sjuksköterskor, undersköterskor, sjukgymnast/lymfterapeut, arbetsterapeut, dietist och kurator. Teamet präglas av engagemang och arbetsglädje och av närhet till patienten och dess anhöriga. För att upprätthålla hög kompetens håller vi interna utbildningar på enheten och är positiva till extern utbildning. Vi har ett reflekterande arbetssätt i våra dagliga dialoger i teamet för en lärande organisation. Vi vårdar svårt sjuka patienter i hemmet dygnet runt. De vanligaste diagnoserna är cancer i olika skeden, multisjuklighet, KOL, Hjärtsvikt Patienterna kan var kurativa, palliativa och i terminal fas. En del patienter avslutar sitt liv i hemmet. Vi ansluter även patienter med korta vårdtider tex under en infektionsbehandling Patienterna remitteras mest från de stora sjukhusen i Stockholm men även från primärvården. Vi har ett väl inarbetat samarbete med de onkologiska klinikerna. Vi dokumenterar i Take Care Friskvård 1 tim/vecka Kollektivavtal Handledning av extern handledare Vem är du? Är du Underläkare eller Leg Läkare och vill lära dig mer inom pallitiv vård? Du kommer arbeta i det patientnära arbetet som utgår från det multiprofessionella teamet med stöd och närhet av en erfaren Specialistläkare. Arbetet innefattar medicinsk bedömning och symtomkontroll med allt vad det innebär av hembesök, läkemedelsjusteringar, kommunikation med kollegor på sjukhusen och närståendesamtal etc
