Svea Boende söker boendestödjare som kan arbeta vid behov på vårt stödboende i Stockholm. Arbetspassen erbjuds utifrån verksamhetens behov, exempelvis vid ordinarie personals frånvaro eller när bemanningsbehov uppstår med kort varsel. Om rollen Som boendestödjare hos Svea Boende arbetar du nära personer som har beviljats en stödboendeinsats enligt socialtjänstens beslut och som kan ha psykosocial problematik, psykisk ohälsa, funktionsnedsättningar eller andra individuella stödbehov. Tillsammans med klienten planerar, genomför och följer du upp stödet utifrån socialtjänstens beställning, klientens genomförandeplan och individuellt uppsatta mål. Rollen kombinerar relationsskapande arbete, motivationsarbete, praktiskt stöd och administrativa uppgifter som dokumentation, journalföring och riskanalyser. Ensamarbete förekommer. Ansvarsområden Arbeta nära klienten för att stärka självständighet och måluppfyllelse Upprätta och följa upp genomförandeplaner enligt socialtjänstens beställning Dokumentera genom journalföring, riskanalyser och månadsrapporter Ha regelbundna veckovisa möten med klienter för att följa upp mående och planering Stödja klienter i kontakt med myndigheter, vårdkontakter och sysselsättning Ge praktiskt stöd i vardagliga situationer som matlagning, städning, planering och enklare ekonomiska frågor Växla arbetssätt och tempo utifrån klienternas individuella behov Arbeta självständigt vid ensamarbete, följa verksamhetens rutiner och göra professionella bedömningar när situationen kräver ett snabbt agerande.
