Hos Bräcke diakoni är kunskap och omtanke om människor vägledande i allt vi gör. Nu söker vi två personligt ombud till vår verksamhet i Göteborg – en roll där du bidrar till att förbättra livsvillkoren för människor med psykisk funktionsnedsättning. Vi söker två personliga ombud för tillsvidareanställning. I samband med rekryteringen kan även vikariat komma att bli aktuellt. Ange gärna om du även är intresserad av vikariat. Tjänsterna är på heltid och tillträde sker enligt överenskommelse. Arbetet är i huvudsak förlagt till kontorstid. Din roll Som personligt ombud är du en viktig kraft för att människor ska kunna hitta rätt i vård, myndighetskontakter och vardagsliv - mot ett självständigare och delaktigare liv i samhället samt en förbättrad livssituation. Du arbetar stödjande, rådgivande och samordnande, utan att fatta beslut eller bedriva behandling. Tillsammans med individen kommer du skapa vägar framåt och stärka dennes möjligheter att själv ta ansvar för sin vardag. Du kommer bland annat att: Kartlägga och tydliggöra uppdragsgivarnas (klienternas) behov inom vård, stöd, sysselsättning, ekonomi och boende. Stötta i kontakter med myndigheter, sjukvård och andra aktörer. Arbeta med systempåverkan genom att synliggöra och rapportera brister som våra uppdragsgivare möter i välfärdssystemen.

Du är strukturerad och självständig, med förmåga att möta människor i utsatta situationer med stabilitet och respekt. Kvalifikationer: Högskoleutbildning som socionom, psykiatrisjuksköterska, beteendevetare, arbetsterapeut eller annan likvärdig utbildning Erfarenhet av arbete med personer med omfattande psykisk funktionsnedsättning God kunskap om samhällets stödsystem och aktuell lagstiftning, exempelvis SoL, LSS, HSL och socialförsäkringslagstiftningen Kulturell och språklig kompetens
