Vill du ha en central roll i att säkerställa att barn, unga och familjer ska få rätt insats samt bidra till en sammanhållen öppenvård? Vi söker nu en engagerad samordnare till Öppenvården barn och unga. Vi ser fram emot din ansökan! ARBETSUPPGIFTER Som samordnare inom Öppenvården leder och fördelar du arbete med målet, rätt insats till rätt klient/familj. Du samverkar med myndighet kring ärendeflöde genom veckovisa avstämningar och bistår handläggarna med tankar kring öppenvårdsinsats. Du har ett nära arbete med samordnare på Öppenvården vuxen för att optimera att våra medborgare får bästa möjliga behandlingsinsats. Vidare samordnar du inflödet av IUB med målet att Tranås invånare ska få råd och stöd kring sociala frågor inom området barn och familj. Du har återkommande avstämningar med behandlare där du följer upp behandlingsprocessen och vägleder behandlarna i deras arbete. Du har veckovisa avstämningar med vårt team för intensiv familjebehandling. Tillsammans med samordnare på Öppenvården Vuxen samt enhetschef arbetar ni för en enad Öppenvård i Tranås. KVALIFIKATIONER Vi söker dig som är utbildad Socionom eller har likvärdig högskoleutbildning, samt erfarenhet av behandlingsarbete med barn och unga. Steg 1-utbildning är meriterande, liksom erfarenhet av vägledning inom intensivt behandlingsarbete. Du har god digital vana. Körkort är ett krav för tjänsten. För att passa i denna roll bör du vara: Strukturerad: Du planerar, organiserar och prioriterar arbetet på ett effektivt sätt. Du sätter upp och håller tidsramar. Relationsskapande: Du är kommunikativ och samarbetar väl med andra. Du tar aktivt initiativ i sociala sammanhang, kliver fram och har lätt för att både skapa nya relationer samt upprätthålla befintliga. Flexibel: Du har lätt att anpassa dig till ändrade omständigheter. Du kan snabbt ändra ditt synsätt och förhållningssätt, och ser möjligheterna i förändringar. Samordnande: Du har förmåga att se både individens behov och den större helheten. Koordinerar eller projektleder andra för att effektivt nå gemensamma mål. Blir en referenspunkt och skapar engagemang och delaktighet. Samarbetsförmåga: Du arbetar bra med andra människor, samt relaterar till dem på ett lyhört och smidigt sätt. Du lyssnar, kommunicerar och löser konflikter på ett konstruktivt sätt.

ARBETSPLATSEN Öppenvården består i dag av en enhetschef, en samordnare och 9 familjebehandlare och enheten har ett gemensamt uppdrag att ge råd, stöd och behandling till barn, ungdomar 0–20 år och deras familjer.
