L'ASPMAD, C'EST : ï€ 9 CMS dans le Nord vaudois ï€ 950 collaborateur\-rice\-s engagé\-e\-s ï€ Plus de 6'700 personnes accompagnées chaque année ï€ Une présence dans 105 communes POUR NOTRE CMS DE COSSONAY, NOUS RECRUTONS UN\-E : ASSISTANT\-E SOCIAL\-E 80% Assistant\-e social\-e Cossonay à 80% VOTRE MISSION En qualité d'assistant(e) social(e), votre rôle implique une maîtrise avancée de compétences spécifiques : ï‚· Analyse des besoins et intervention sociale : Vous identifiez et évaluez les besoins spécifiques des clients afin de diriger des interventions dans un cadre de santé sociale adapté, incluant un suivi personnalisé en fonction des besoins de chaque client. ï‚· Support aux proches\-aidants et orientation : Vous fournissez conseil et soutien aux proches, tout en offrant des orientations stratégiques pour améliorer le bien\-être des clients. ï‚· Collaboration interdisciplinaire : Vous collaborez efficacement au sein d'une équipe pluridisciplinaire et avec un réseau externe pour une approche coordonnée. ï‚· Gestion de cas : Vous gérez activement des dossiers clients en agissant comme référent\-e VOTRE PROFIL ï‚· Vous détenez un diplôme d'assistant(e) social(e) ou un titre reconnu comme équivalent, complété par idéalement deux années d'expérience en Suisse. ï‚· Vous avez une solide connaissance des assurances sociales du système socio\-sanitaire du canton de Vaud. ï‚· Une formation certifiante en assurances sociales constituerait un plus. ï‚· Doté(e) d'autonomie, de polyvalence, et d'une facilité d'adaptation, vous avez un intérêt marqué pour le maintien à domicile ainsi que les assurances sociales et la relation d'aide. ï‚· Vous présentez une grande capacité d'écoute et de communication au sein d'un groupe et auprès des clients. ï‚· Vous maîtrisez les démarches administratives avec aisance et assurance ! ï‚· Vous possédez un permis de conduire et, idéalement, un véhicule personnel.
