Highlights:
Summary: The Transition of Care Nurse is responsible for managing the inception of transition of care document and validation of data that is received. This includes developing a comprehensible transition into the physician practice or into a higher level of care. The transition of care (TOC) document ensures that all providers are working on the same information to ensure continuity of care. Data is also validated through phone conversation with the patient once an appointment has been made in the system. Education / Experience:Degree in Nursing required, Bachelor's degree preferred. Minimum 2 years staff RN experience. Experience with quality measures/metrics preferred. License / Certification: Active RN licensure required, BLS required Knowledge, Skills, and Abilities: • Evidence to commitment to continuing education. • Ability to work both independently and in a team environment. • Ability to foster a cooperative work environment. • Ability to work from broad directions and be self motivated. • Ability to prioritize workload to achieve maximum efficiency. • Ability to effectively communicate with people of various backgrounds throughout the organization. • Ability to provide a positive customer service experience to guest, visitors, and patients. • Knowledge of basic Windows based PC operations. • Excellent organizational skills. • Strong analytical skills. • Exceptional oral and written communication skills.
Working Conditions/Physical Requirements: Position requires sitting for 80% of the workday and mobility 20% of the workday. There is occasional stooping, bending, kneeling, and crouching. There is frequent reaching, use of hands and repetitive action. Requires occasional lifting or carrying up to 35 pounds, pushing/pulling up to 150 pounds. There is a continuous need for speaking with clear diction and hearing sensitivity in conversation either by phone or in person. Continuous visual acuity at close and distant range. Inside protected from weather conditions but not necessarily from temperature changes. Minimal noise levels. Much exposure to computer work. Occasional exposure to risk of electrical shock. Some exposure to infectious disease while performing job-related tasks. Some exposure to hazardous chemicals. Occasional exposure to radiation.
