Summary:
The Care Team Navigator (CTN) acts as the primary point of contact between the patient living with dementia (PLWD), their caregiver (referred to as the dyad), and the care team at the Baystate Memory Assessment and Care Center (MACC). Much of this work is by video or telephone appointments. The CTN role does not require a college degree or background experience in health care. Excellent communication skills are a must. Fluency in Spanish is desired, but not required. CTNs will receive training in ethics, aging, dementia care, and advanced care planning. In addition to having access to a large resource library, CTNs will receive continuing and ongoing support from members of the clinical care team who have formal medical training in dementia care, patient management, nursing, pharmacy, and social work.
