Integrated Behavioral Health Care Social Worker opportunity in beautiful Hamilton, Montana!! Bitterroot Health is seeking a licensed social worker to serve as the perinatal behavioral health clinician and care coordinator for The Meadowlark Initiative®. This position embeds social work and integrated behavioral health into prenatal, postpartum, and Labor & Delivery care so that moms and babies stay healthy and families stay together. The Meadowlark model of care combines universal screening for depression, anxiety, substance use, and social determinants of health with same-visit or same-day warm hand-off to a behavioral health clinician, brief evidence-based intervention, care coordination, and navigation to a community team of trusted local and state resources. This role is the clinical-community bridge on the Meadowlark team. The social worker works side by side with obstetric, family medicine, midwifery, and L&D providers; partners with the psychiatric consultant as available; and builds the community partnerships that make the model work in Ravalli County. Bitterroot Health will support an unlicensed or candidate social worker in obtaining Montana licensure (LSW or LCSW), including supervision time, application support, and examination leave as operationally feasible. Minimum Qualifications: Education:
Bachelor’s degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) from a CSWE-accredited program strongly preferred. Related degrees (psychology, counseling, human services, public health) may be considered when paired with substantial social-work or perinatal behavioral health experience.
Licensure
Montana LCSW, LSW, LCPC, LMFT, or LAC preferred. Licensure candidates and unlicensed social workers are eligible. Bitterroot Health will help the selected candidate obtain and maintain Montana social work licensure, including arranged clinical supervision toward LSW/LCSW when the candidate holds a qualifying degree. Must be willing to apply for Montana licensure (or candidate status) within 60 days of hire if not already licensed.
Experience
One year of professional experience in social work, behavioral health, care coordination, or a related field preferred. Experience screening for common mental health and/or substance use disorders preferred. Experience with perinatal, prenatal, postpartum, or women’s health populations strongly preferred. Experience with assessment and treatment planning for common mental health and/or substance use disorders preferred. Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions (problem-solving treatment, behavioral activation, motivational interviewing, SBIRT) preferred. Knowledge of social determinants of health, community resource navigation, child welfare systems, and family-centered practice preferred. Basic knowledge of psychopharmacology for common mental health disorders preferred.
