Assessment Field Support Registered Nurse (RN / AFSN) - Full Time | Hourly | Monday-Friday, Day shift | Pay Range $32-$38/hr | 32-36 hours/weekly

BrightStar Care’s provision of nurse oversight for individuals receiving caregiving services supports a more comprehensive, well-rounded approach to care. Nursing oversight allows for ongoing assessment of the client’s health status, identification of changes or emerging concerns, and timely intervention when a change in condition is identified. This collaborative approach helps promote client safety, continuity of care, and overall well-being while supporting caregivers in meeting the client’s evolving needs.

Position Summary

The Registered Nurse / RN role is responsible for assessments, in addition to providing oversight and support for BrightStar Care clients receiving caregiving services. Reporting directly to the Director of Nursing (DON), this role serves as a clinical resource for clients, families, and caregivers by ensuring quality care, regulatory compliance, and continuity of services.

The role conducts initial admission assessments and ongoing reassessments, supports the admission process, provides education and competency validation, and partners with the Director of Nursing and Director of Operations on quality improvement initiatives. This position is highly visible in the field, serving as a liaison between clients, caregivers, and BrightStar Leadership to ensure exceptional care and satisfaction.

This position is not responsible for employee performance management, disciplinary actions, scheduling, or direct supervision of staff.

Essential Responsibilities

Clinical Oversight

  1. Complete comprehensive initial assessments for new clients receiving caregiving home care services.
  2. Perform scheduled reassessments and supervisory visits in accordance with agency policy, regulatory guidelines, Joint Commission recommendations, physician orders, and/or payer requirements.
  3. Develop, review, and update individualized plans of care and task lists.
  4. Identify changes in client condition and communicate recommendations to the Director of Nursing, Director of Operations, and other members of the care team.
  5. Ensure services are delivered safely and in accordance with clinical standards and best practice, BrightStar Care policies, and Joint Commission recommendations.

Patient & Family Support

  1. Assist with the admission process and provide a smooth transition for new clients and families into BrightStar Care services.
  2. Educate clients and families regarding care plans, expectations, safety, and available resources.
  3. Serve as a trusted clinical resource for clients, families and caregivers.
  4. Collaborate with the Director of Nursing and Director of Operations to resolve issues and promote positive client outcomes.

Caregiver Support

  1. Provide support to caregivers in the client homes.
  2. Conduct caregiver skills validation and new hire competency check-offs.
  3. Identify educational opportunities and provide coaching, mentoring, and training.
  4. Support ongoing competency and best practices through home visits and one-on-one education.
  5. Serve as a resource for caregivers requiring assistance with patient care concerns.

Quality & Compliance

  1. Participate in quality assurance and performance improvement initiatives.
  2. Assist with chart audits to ensure documentation accuracy, completeness, and regulatory compliance.
  3. Support agency readiness for Joint Commission accreditation and regulatory surveys.
  4. Monitor compliance with agency policies, infection prevention standards, and documentation requirements.
  5. Collaborate with the Director of Nursing and Director of Operations to identify process improvements that enhance client care and outcomes.

Collaboration

  1. Communicate client status updates and concerns promptly to the Director of Nursing and Director of Operations.
  2. Foster positive working relationships among clients, families, caregivers, physicians, and community partners.
  3. Act as a clinical liaison between caregiving staff, clients/families, and agency leadership.

Qualifications

  1. Current unrestricted Registered Nurse (RN) license in the state of practice.
  2. Minimum of two years of nursing experience caring for adults or geriatric populations; home health, hospice or community-based care preferred.
  3. Previous experience performing physical assessments, developing care plans, and assessing social determinants of health (SDOH).
  4. Excellent communication, teaching, and relationship-building skills.
  5. Strong organizational and critical thinking abilities.
  6. Proficient in EMR documentation, as well as Microsoft Office applications and other standard office technology.
  7. Ability to work independently while collaborating effectively with interdisciplinary teams.
  8. Valid driver's license, reliable transportation, and ability to travel within the service area.

Physical Requirements

  1. Travel to client homes and community locations.
  2. Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull with client assessments as needed. Must be able to lift up to 50 pounds. Visual and hearing acuity required.
  3. Ability to utilize standard office equipment and electronic documentation systems.

Through comprehensive assessments, quality monitoring, caregiver competency validation, and a strong field presence, this role helps ensure every BrightStar Care client receives safe, compassionate, and high-quality care while supporting caregivers in delivering excellence in the home.
