Capio Vårdcentral Kvillebäcken söker en engagerad diabetessjuksköterska med patientens behov i fokus. Hos oss blir du en viktig del av en verksamhet där kvalitet, trygghet och ett gott bemötande står i centrum. Vi arbetar tillsammans för att ge våra patienter en god och säker vård, med patientens behov i fokus.

Din roll

Som diabetessjuksköterska hos oss självständigt samt med teambesök, planerade mottagningsbesök och i nära samarbete med läkare och andra professioner för att skapa en sammanhållen vård.

Arbetsuppgifterna omfattar bland annat:

  • Mottagningsarbete och uppföljning av patienter med diabetes
  • Omvårdnadsbedömningar, rådgivning och patientutbildning
  • Teambesök
  • Teamarbete
  • Kontakt med remittenter och samverkan med andra vårdgivare
  • Medverkan i utvecklings- och förbättringsarbete inom diabetesvård och primärvård

Arbetet kan, utifrån verksamhetens erbjudande och patienternas behov, ske genom fysiska möten och digitala vårdkontakter via exempelvis telefon, chatt och video.

Om dig

Vi söker dig som är legitimerad sjuksköterska med vidareutbildning i diabetesvård.

Erfarenhet från primärvård, mottagningsarbete, kronikeruppföljning och digitala vårdmöten är också värdefull.

Du har god förmåga att prioritera och fatta beslut i det dagliga arbetet. Du arbetar enligt gällande lagar, riktlinjer och rutiner och bidrar aktivt till en trygg och säker vård.

Goda kunskaper i svenska i tal och skrift är ett krav. Vi lägger stor vikt vid personlig lämplighet.
