Unser Kunde sucht ab 01. August 2026 für mindestens 3 Monate - mit Option auf Verlängerung, eine Dipl. Pflegefachperson HF/ FH für Einsätze zu Hause. Die gemeinnützige Organisation unterstützt Menschen in somatischer und Psychiatriepflege sowie Palliative Care. In dieser temporären Aufgabe übernehmen Sie wichtige Pflegeleistungen, begleiten den Alltag der betreuten Personen und sorgen für Sicherheit.

Ihre Aufgaben beim neuen Arbeitgeber:

  • Sie pflegen und betreuen unsere Klientinnen und Klienten direkt in deren Zuhause und gewährleisten eine individuelle und würdevolle Versorgung
  • Sie übernehmen die Verantwortung für die professionelle Planung und Durchführung des gesamten Pflegeprozesses mit Fallführung
  • Sie ermitteln Pflege- und Betreuungsbedarf mit dem interRAI HC-Instrument, führen Reassessments durch und passen die Pflegeplanung bedarfsgerecht an
  • Sie sind Ansprechperson für Angehörige, Hausärzte, Apotheken sowie Sozialen Dienstleistern und wirken bei Kooperation und Koordination mit

Sie bieten Ihrem neuen Arbeitgeber:

  • Sie verfügen über eine abgeschlossene Ausbildung als dipl. Pflegefachperson HF / FH
  • Sie sind eine reflektierte, aufgeschlossene und flexible Persönlichkeit mit Empathie, Geduld und der Fähigkeit, auch in schwierigen Situationen den Überblick zu behalten
  • Sie arbeiten selbstständig, strukturiert und mit hoher Eigenverantwortung - ideal für Einsätze mit hoher eigen Verantwortung
  • Sie besitzen gute Deutschkenntnisse in Wort und Schrift sowie einen Führerausweis der Kategorie B
