Job Description Summary

‎

The Director, Extended Care Operations leads the operational strategy implementation, design, standardization, and performance of CareMore’s non-clinic-based Extended Care programs across markets. Acting independently within the strategic direction established by senior clinical and business leadership, this role makes functional operating decisions, establishes operational policies and standards, and resolves highly complex, ambiguous issues spanning clinical teams, markets, care settings, and external partners.

In partnership with the Extended Care Medical Director, the Director is accountable for operational performance, resource deployment, service reliability, and financial stewardship that support measurable improvements in care delivery for high-risk and frail patients. The role directly leads the Extended Care Coordinator team and influences clinical and cross-functional leaders responsible for program execution.

‎

How will you make an impact & Requirements

‎

Core Responsibilities

1. Functional Operating Model and Strategy Implementation

· Lead design and implementation of standardized operating models across Extended Care markets and programs, translating CareMore strategy into functional policies, operating plans, and measurable standards.

· Establish operational decision rights, roles, escalation protocols, service levels, governance, and cross-setting handoffs; determine and implement changes within delegated authority.

· Resolve ambiguous cross-functional operating issues and drive adoption of new practices across care centers, network partners, hospitals, SNFs, and support services.

2. Workforce Leadership and Capacity Decisions

· Direct the central Extended Care Coordinator team, including hiring recommendations or decisions within delegated authority, performance management, training, succession and staffing plans.

· Own workforce capacity planning for coordinator support and recommend or approve resource deployment and headcount changes within established budget and approval limits.

· Partner with clinical leaders to establish clinician coverage and productivity operating standards; influence execution across teams without implying direct supervisory authority over clinicians.

3. Care Coordination and Operational Governance

· Direct operating standards for referral intake, patient assignment, outreach, tracking, discharge, and escalation across programs.

· Establish and monitor reliable patient-transition workflows across hospitals, post-acute facilities, home-based care, and primary care teams.

· Make timely cross-program operational decisions to address service gaps, handoff failures, and competing resource demands within delegated authority.

4. Performance, Budget and Vendor Accountability

· Set operational performance targets and measurement approaches across Extended Care programs and lead corrective actions when performance is below standard.

· Manage the department operating budget; make material spending and resource allocation recommendations and decisions within approved authority, and explain financial tradeoffs to senior leaders.

· Partner with clinical leaders and analytics to assess utilization, cost per patient, program effectiveness, and ROI; use findings to adjust operating models.

· Oversee Extended Care vendors, establish service expectations, evaluate performance, and lead operational remediation or escalation.

5. Cross-Functional Leadership and Transformation

· Influence senior leaders outside the immediate reporting line to align priorities, adopt standardized practices, and resolve dependencies across markets.

· Lead complex operational transformation initiatives with defined milestones, risks, resource plans, and measurable outcomes over annual and multi-year planning cycles.

· Present recommendations, performance trends, risks, and investment tradeoffs to clinical and business leadership.

Operational outcomes

· Care at Home referral conversion and time to establish care; ISNP enrollment and retention.

· Facility-team alignment to high-density hospitals and SNFs; ISNP visit cadence and productivity.

· Care at Home episodic and longitudinal panel management and visit productivity.

· Coordinator task completion, volumes, service levels, and handoff reliability.

· Department operating expense, cost per patient served, resource utilization, and vendor performance.

Clinical outcomes (in partnership with clinical leadership)

· ISNP hospitalizations, readmissions, and Stars/BOI gap closure.

· SNF average length of stay and 30-day return-to-acute rate.

· Supportive Care engagement and appropriate hospice transitions.

· Care at Home impact on high-risk/frail patient hospitalizations.

Qualifications

Required

· Bachelor’s degree in healthcare administration, business, or related field.

· 8+ years of progressive healthcare operations, population health, or strategic operations experience, including 3+ years in leadership.

· Experience designing, scaling, and governing healthcare programs or operating models across multiple teams or markets.

· Demonstrated success leading multidisciplinary teams through highly complex organizational change and influencing senior stakeholders.

· Experience managing operational performance, budgets, workforce planning, resource tradeoffs, and program evaluation.

· Strong analytical judgment, executive communication, and cross-functional leadership skills.

Preferred

· Master’s degree in healthcare administration, business, or related field.

· Experience in delegated-risk medical groups or other risk-bearing healthcare organizations.

· Experience with high-risk Medicare populations, care transitions, home-based care, or facility-based services.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
