Die Caritas-Klinik Dominikus verbindet moderne Medizin mit einer menschlichen und patientennahen Versorgung. In unseren Bereichen der Innere Medizin und Geriatrie betreuen wir täglich Patient:innen mit komplexen internistischen und diabetologischen Krankheitsbildern.

Zur Erweiterung unseres Teams suchen wir eine engagierte Pflegefachkraft mit Interesse an der diabetologischen Versorgung oder bereits vorhandener Qualifikation im Bereich der diabetologischen Pflege.

Innere Medizin / Geriatrie | ab sofort | Teil- bis Vollzeit

  • Spezialisierung: Aktive Mitarbeit im Bereich der diabetologischen Versorgung sowie Mitwirkung bei Schulungs- und Beratungsangeboten
  • Interdisziplinarität: Enge Zusammenarbeit mit Ädem ärztlichen Dienst, Ernährungstherapeut:innenund weiteren Fachbereichen
  • Geriatrischer Fokus: Ganzheitliche und bedarfsgerechte Betreuung älterer Patient:innen mit Diabetes mellitus
  • Entwicklung: Gezielte Unterstützung bei Fort- und Weiterbildungen im diabetologischen Bereich zur fachlichen und persönlichen Weiterentwicklung
  • Teamkultur: Wertschätzendes, kollegiales und multiprofessionelles Arbeitsumfeld

Deine Aufgaben

  • Betreuung: pflegerische Versorgung und ganzheitliche Begleitung von Patient:innen mit Diabetes mellitus im klinischen Alltag
  • Schulung: Mitwirken bei der Durchführung diabetologischer Schulungen sowie Beratungen zur Anleitung im Umgang mit verschiedenen Therapieformen (z.B. Insulintherapie, etc.)
  • Überwachung: Regelmäßige Kontrolle von Blutzuckerwerten sowie Beobachtung und Bewertung des Therapieverlaufs
  • Zusammenarbeit: Enge interdisziplinäre Abstimmung mit Ärzt:innen und weiteren therapeutischen Berufsgruppen sowie Teilnahme an Teambesprechungen und Fallkonferenzen
  • Dokumentation: Strukturierte Dokumentation aller pflegerischen und diabetologischen Maßnahmen
