POSITION SUMMARY
The Pharmacist provides safe, accurate, efficient, and patient-centered pharmacy services within Desert Sage Health Centers’ in-house pharmacy. Responsibilities include prescription verification and dispensing, patient counseling, medication safety, medication access, oversight of pharmacy technicians, and other professional pharmacy services within the pharmacist’s scope of practice.
The Pharmacist works collaboratively with the Pharmacy Manager / Pharmacist-in-Charge (PIC), DSHC providers, clinical teams, and pharmacy staff to improve medication access, support continuity of care, and promote appropriate use of pharmacy services. DSHC pharmacists are also expected to use their clinical pharmacy knowledge to identify and support opportunities to improve medication therapy, adherence, safety, and patient outcomes.
SUPERVISION RECEIVED
Works under the direct supervision of the Pharmacy Manager / Pharmacist-in-Charge. Exercises independent professional judgment within the pharmacist scope of practice and provides professional supervision of pharmacy technicians and support personnel as assigned.
PRIMARY DUTIES AND RESPONSIBILITIES
- Reviews, verifies, prepares, and dispenses prescriptions accurately and safely in accordance with applicable laws, professional standards, and DSHC policies and procedures.
- Provides medication counseling and education to patients regarding appropriate medication use, adherence, precautions, interactions, side effects, and other medication-related concerns.
- Reviews medication therapy for safety and appropriateness, identifies potential medication-related problems, and collaborates with prescribers and clinical staff when intervention or clarification is needed.
- Provides professional supervision and direction to pharmacy technicians and pharmacy support staff during assigned shifts.
- Works with patients, providers, insurance plans, and pharmacy staff to resolve prescription, coverage, prior authorization, medication access, and other pharmacy-related barriers.
- Performs and supports compliant 340B pharmacy workflows in accordance with DSHC policies and procedures and reports identified concerns or discrepancies to the Pharmacy Manager / PIC.
- Supports pharmacy operations including prescription workflow, inventory management, controlled substances, recalls, medication shortages, recordkeeping, pharmacy security, and medication safety.
- Collaborates with DSHC clinical teams to support medication reconciliation, adherence, medication access, chronic disease management, and other opportunities where pharmacist expertise can improve patient care.
- Participates in clinical pharmacy services appropriate to DSHC patient needs and organizational priorities, which may include immunizations, medication management, point-of-care services, collaborative pharmacy services, or other activities within the pharmacist’s scope of practice.
- Provides courteous, responsive service to patients and promotes appropriate utilization of DSHC pharmacy services while preserving patient choice and clinical appropriateness.
- Participates in pharmacy quality improvement, workflow improvement, training, and other pharmacy initiatives as assigned.
- Demonstrates DSHC’s core values and performs other duties as assigned.
MINIMUM QUALIFICATIONS
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited school of pharmacy.
- Current Idaho pharmacist license in good standing, or ability to obtain Idaho licensure before beginning pharmacist duties.
- Knowledge of community, retail, ambulatory, or outpatient pharmacy practice, including prescription dispensing, patient counseling, medication safety, and applicable pharmacy requirements.
- Demonstrated ability to exercise sound clinical judgment, communicate effectively with patients and healthcare professionals, and perform detailed work accurately.
- Ability to work collaboratively with prescribers and interdisciplinary healthcare teams.
PREFERRED QUALIFICATIONS
- Previous pharmacist experience in a community, retail, ambulatory, or outpatient pharmacy.
- Experience with 340B pharmacy operations, an FQHC, community health center, or similar healthcare setting.
- Experience with Medicaid, Medicare, commercial prescription insurance, PBMs, prior authorizations, and medication-access programs.
- Experience or demonstrated interest in clinical pharmacy services such as medication management, adherence support, immunizations, chronic disease management, or collaborative pharmacy practice.
WORK ENVIRONMENT
Work is performed primarily in a licensed outpatient pharmacy and healthcare clinic environment with routine interaction with patients, employees, medications, pharmacy equipment, technology systems, providers, and other healthcare partners.
TYPICAL PHYSICAL DEMANDS
This position requires the ability to stand and walk for extended periods, work at a computer, communicate effectively with patients and staff, reach and handle medications and pharmacy supplies, and lift or carry items commonly encountered in pharmacy operations.
