Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking compassionate Registered Nurses (RNs) to join our team and provide exceptional end-of-life care and support to patients and their families in the comfort of their own homes. We are currently hiring Hospice Registered Nurse Case Managers in the following areas:
Beaver County (lower) Moon Township / McKees Rocks / South Hills Monroeville/Cheswick/Oakmont areas

As a Hospice Registered Nurse Case Manager, you will be an integral member of our interdisciplinary team, working closely with physicians, social workers, spiritual care, volunteers, patients, and families to ensure comfort, dignity, and quality of life throughout the hospice journey. Qualifications
Current Registered Nurse (RN) license in Pennsylvania Strong clinical assessment and nursing skills Excellent computer and documentation skills Strong organizational skills and attention to detail Excellent written and verbal communication skills Reliable independent transportation Valid Pennsylvania driver's license
Responsibilities As a Hospice Registered Nurse Case Manager, you will:
Assess and monitor the physical, emotional, and spiritual needs of hospice patients and develop individualized plans of care. Provide compassionate nursing care focused on comfort, dignity, and quality of life. Educate patients and families regarding the disease process, medications, care options, and end-of-life decisions. Provide pain and symptom management to promote comfort and quality of life. Coordinate care with physicians and members of the interdisciplinary hospice team. Serve as a patient advocate, ensuring patients' wishes, preferences, and goals of care are respected. Support and educate family members and caregivers throughout the hospice experience. Communicate changes in patient condition and collaborate with the care team to ensure appropriate interventions. Complete timely and accurate clinical documentation.
