About the role Working within a multidisciplinary team, you will help public housing residents impacted by relocation to access health, social and community supports, ensuring they remain connected and supported throughout the move and in their new communities. This rewarding role combines outreach, client engagement, service coordination, case management, and community connections to improve wellbeing and achieve positive relocation outcome This position requires applicants to work onsite and does not have hybrid working arrangements. Please refer to the Position Description to know more about the role. Your duties include:

Complete intake assessment forms and client registration in TRAK, identifying barriers residents face engaging with the service Assist residents with non-clinical relocation needs responding to gaps in resident knowledge and understanding the program and relocation process Identify appropriate engagement and promotion opportunities with residents to help build program awareness

Our ideal candidate:

Tertiary qualification in Community Development, Social Work, Public Health, or related field or 5 years relevant experience Strong interpersonal and stakeholder engagement skills Ability to work independently while supporting a multidisciplinary team Experience working with culturally diverse communities and people with complex needs
