Community Care is hiring a Quality Management Coordinator. This is a full-time opportunity at our Bishop Woods facility. Quality Management Coordinator Job Responsibilities: Under the supervision of the Director of Quality, the Quality Management Coordinator is responsible for supporting the operational quality of the organization. The Quality Management Coordinator also Uses quality monitoring systems to ensure accuracy and completeness of program data.
Quality Monitoring: Conducts internal file reviews as assigned to monitor program performance and adherence to quality standards. Completes additional monitoring audits as assigned. Supports data analysis and reporting assistance with production of data for external review as needed. Risk Management: Monitors the Incident Management System to include receiving, responding and tracking member related events reported by Program Operations. Completes incident investigations as defined by policy. Consults with staff to ensure proper remediation for the member and the organization with the goal of meeting member outcomes, ensuring member safety and minimizing risk. Quality Program Support: Supports performance improvement initiatives as assigned. Quality Program Engagement Seeks and integrates stakeholder input into quality assurance and risk related initiatives. Stakeholders include members, legal decision makers, care teams, caregivers, and providers
Quality Management Coordinator Experience: A Bachelor's degree in Healthcare Administration or Human Service with 2 years of experience combining direct care/service in acute or community base long term healthcare with older adults and/or persons with disabilities. or an Associate's degree with 4 years of experience combining direct care/service in acute or community base long term healthcare with older adults and/or persons with disabilities Quality Management Coordinator Education: Bachelors Degree in Healthcare Administration or Human Service with 2 years of experience required or Associates Degree in Healthcare Administration or Human Services with 4 years of experience required.License/Certification: Long Term Functional Screen Certification may be required based on specific assignment. Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
