Führend bei Erkrankungen des Bewegungsapparates, in der Inneren Medizin (Kardiologie, Gastroenterologie, Nephrologie), Neurologie, Altersmedizin, Schmerzmedizin und Psychosomatik. Ein eigenes Institut dient Ausbildung, Wissensaustausch und beruflicher Weiterentwicklung. In Ambulanzen, Tageskliniken und stationären Klinikbereichen arbeiten mehr als 1.000 Mitarbeitende in multiprofessionellen Teams und machen moderne Medizin für Menschen.

Unsere Klinik für Psychosomatische Medizin und Psychotherapie bietet Menschen mit sämtlichen psychosomatischen Erkrankungen ein multimodales und multiprofessionelles Therapieangebot, das stationär, tagesklinisch und ambulant durchgeführt wird.

Bereichern Sie unser Team als Sozialarbeiter Psychosomatik (m/w/d)

Ihre Aufgaben:

  • Unterstützung - Sie sind zuständig für die sozialarbeiterische Unterstützung stationärer Patient:innen.
  • Entlassmanagement planen – Sie organisieren Nachsorge, Hilfen im Alltag, Wohn-/ Arbeits­perspektiven, Rehabilitationsmaßnahmen und Übergänge.
  • Netzwerke verknüpfen – Sie koordinieren Kooperationen mit Behörden, Kostenträgern, Betreuungen, Ambulanzen und Hilfeangeboten.
  • Dokumentation sichern – Sie dokumentieren strukturiert, erstellen Berichte und wirken an Qualitätsprozessen mit.

Qualifikationen

Sie vervollständigen unser Team, wenn Sie...

  • ein abgeschlossenes Studium (Soziale Arbeit) oder eine abgeschlossene Ausbildung zur Sozialarbeiterin / zum Sozialarbeiter mitbringen.
  • Berufserfahrung in der psychosomatischen / psychiatrischen Arbeit und psychosozialen Beratung haben.
  • flexibel, empathisch und strukturiert arbeiten und gern im Team wirken.
  • Freude an der Arbeit in einem engagierten, interdisziplinären Team haben.
  • Eigeninitiative zeigen, kommunikativ und persönlich gefestigt auftreten.
