Er du social- og sundhedsassistent med et godt overblik, gode kommunikative evner og erfaring med planlægning? Så er det dig, vi søger!

Grundet en omorganisering på Ældreområdet i Favrskov Kommune søger vi pr. 1. december 2026 en social- og sundhedsassistent til koordinatorfunktion i hjemmeplejen, Hadsten.

Stillingen er en fuldtidsstilling, hvor koordinatorfunktionen vil udgøre ca. fire timer af den daglige arbejdstid. Den øvrige tid vil du være udekørende med opgaver inden for helhedsplejen. Du vil desuden skulle have ca. 6 aftenvagter på 12 uger samt weekendvagt hver 4. weekend

Som koordinator har du bl.a. ansvar for

  • den daglige planlægning og koordinering i det team, du er tilknyttet samt overordnet koordinering mellem alle teams. Der vil være tale om planlægning af ældrelovsforløb samt ydelser inden for både sundhedsloven og serviceloven.
  • at udarbejde kørelister, der sikrer kontinuitet, og at opgaver løses med rette kompetencer
  • at sikre en stabil drift, herunder dækning af vagter ved fx sygdom
  • at besvare opkald fra borgere, pårørende og øvrige samarbejdspartnere

Vi lægger stor vægt på det tværfaglige samarbejde omkring borgerne og er organiseret i 3 faste teams bestående af både sygeplejersker, social- og sundhedsassistenter, social- og sundhedshjælpere samt ufaglærte. Vi arbejder med helhedspleje og velfærdsteknologi og altid med et rehabiliterende og vedligeholdende sigte.

  • har kørekort til personbil.
