We're hiring a Licensed Practical Nurse (LPN) to serve as a primary provider of resident care in our Assisted Living or Memory Care community. This role reports to the Director of Health & Wellness.

What You'll Do

  • Provide direct resident care services and follow each resident's individualized plan of care
  • Administer medications following standards of practice, state regulations, and required documentation
  • Perform resident assessments as required by state standards and facility policy
  • Observe residents for changes in condition and report immediately to the supervisor or Department Director
  • Document observations, communications, and daily care by the end of each shift
  • Complete and maintain individualized care plans; attend interdisciplinary care meetings
  • Check and record vital signs, weights, and blood sugars as assigned
  • Assist with supervision, coaching, and training of direct care staff
  • Support fall prevention and a restraint-free environment

What You'll Need

  • High school diploma or equivalent (GED) required
  • Licensed Practical Nurse license in applicable state required
  • Current CPR certification required
  • At least 2 years of related work experience
  • Knowledge of medical terminology, medical records management, and clinical/diagnostic procedures

Physical Requirements

  • Ability to sit, stand, walk, climb stairs, reach, turn, bend, stoop, crouch, kneel, and perform fine manipulation throughout the workday
  • Ability to lift up to 25 pounds unassisted and to carry, transfer, push, pull, and reposition residents with assistance or equipment
  • Normal or corrected vision and hearing, including color recognition

Working Environment

  • Work is performed in a well-lit, well-ventilated community setting, with occasional exposure to dust, fumes, odors, water, and noise
  • Occasional outdoor exposure to weather and temperature changes
  • Frequent interruptions and interaction with residents who may be emotionally distressed
  • Flexibility to work beyond normal hours, on weekends, or in other roles as needed
