Home Health Clinical Team Manager (RN)

Full Time/ La Grange

Lead a Team. Support Your Nurses. Make a Lasting Impact.

Your nursing experience can make a difference beyond each patient visit. At Elara Caring, you’ll guide a team of field nurses, help solve clinical challenges, and ensure patients receive the care they need in the place they call home.

We’re looking for an RN Clinical Team Manager (CTM) who brings sound clinical judgment, a supportive leadership style, and a commitment to quality care. You’ll help your nurses grow, maintain clinical standards, and turn opportunities for improvement into meaningful results.

What You’ll Do

  • Lead and develop your team: Provide direction, coaching, and ongoing support to field nurses.
  • Guide patient care: Address clinical concerns and recommend appropriate interventions.
  • Support quality and compliance: Review nursing visits and documentation for accuracy and alignment with state, federal, and contractual requirements.
  • Manage team performance: Help nurses meet productivity expectations while maintaining quality care.
  • Drive improvement: Support Quality Assurance and Performance Improvement (QAPI) initiatives that strengthen clinical outcomes.
  • Keep communication connected: Ensure timely documentation, follow-up, and communication with the appropriate care partners and authorities.

What You’ll Bring

  • Current RN license
  • At least 3 years of active RN experience, including 1 year in home health
  • Knowledge of Medicare/Medicaid home care benefits, policies, and procedures
  • Strong clinical judgment, communication, and organizational skills
  • A commitment to coaching nurses and delivering patient-centered care
  • Valid driver’s license, insurance, and reliable transportation
  • Ability to sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
