Kenmerken: - regio Leiden - 24 uur - Reageren tot 19 oktober 2026 Als aanmeldfunctionaris bij Jados ben jij het warme, eerste aanspreekpunt voor onze nieuwe cliënten en hun netwerk. Je zorgt voor een zorgvuldige beoordeling en een soepel aanmeldtraject. Vanuit inhoudelijke kennis en verbinding denk je actief mee over de best passende ondersteuning. - Regio: Noord West (Leiden - Amsterdam - Den Haag) - Functie: Aanmeldfunctionaris voor 24 uur - Salaris: tussen de € 3.560,-,- en € 4.716,- Wat ga je doen? In een hecht team van zes aanmeldfunctionarissen vorm jij de 'voordeur' van Jados. Je coördineert het aanmeldtraject van nieuwe cliënten binnen jouw regio. - Je begeleidt het gehele proces van de eerste aanmelding tot en met plaatsing of archivering. - Je bent het centrale aanspreekpunt voor de cliënt, het netwerk, verwijzers en gemeenten. - Je bent regelmatig fysiek aanwezig bij regionale veldtafels (onder andere bij de gemeente Amsterdam) om de lijnen kort te houden. - Je beoordeelt het dossier en maakt een zorgvuldige inschatting of de begeleiding van Jados aansluit bij de hulpvraag van de cliënt. - Je werkt intern nauw samen met de regiomanager, gedragswetenschappers, teamleiders en het team klantcontact. - Je zet jouw kennis over onze doelgroep, zorgvisie, locaties en wetgeving (zoals de Wmo en indicatiestelling) in om kandidaten volledig en correct te informeren. - Je deelt je werkweken flexibel in: je werkt vanuit huis of op locatie in de regio Noord West, en je sluit één keer per maand aan bij het fysieke overleg met het regioteam in Leiden.
