Als ambulant begeleider is het belangrijk om de klant in zijn of haar kracht te zetten, zodat eventuele bagage lichter wordt zonder deze zelf over te nemen. Soms loop je mee en motiveer je, soms leer je de klant om de bagage efficiënter te dragen, en soms vind je samen een manier om het los te laten. Als ambulant begeleider in Rijswijk bezoek je klanten in hun thuissituatie waarbij je hen één op één begeleiding biedt. Onze klanten zijn 18 jaar en ouder en hebben te maken met verschillende problemen. Het kan gaan om somatische, psychogeriatrische, psychosociale, culturele en financiële problematiek of opvoedings- en/of verslavingsproblematiek. Dat maakt het werk zo afwisselend! Samen met de klant maak je een begeleidingsplan. Hierin zet je de doelen waar jullie aan gaan werken. De doelen zijn opgesteld om de persoonlijke situatie te verbeteren zodat de klant zo gelukkig mogelijk thuis blijft wonen. Dit plan stuur je op naar de gemeente, waarna je goed contact onderhoudt met de wmo-consulent. Ook begeleid je klanten vanuit de wet langdurige zorg. Dit zijn klanten waar jij stabiliteit biedt en kijkt waar zij met hun chronische beperking of aandoening gelukkig van worden. Als ambulant begeleider ga je uit van de eigen kracht van de klant en je betrekt hierbij het sociale netwerk, andere hulpverleners en instanties. Zo werk je bijvoorbeeld samen met bewindvoerders, huisartsen of dementieconsulenten om de zorg ron...
