- Middin - 32 - 36 uur - Den Haag ( Zuid-Holland ) - 2893 - 3902 - Online sinds: vandaag Middin Help jij mensen met een licht verstandelijke beperking (lvb) of niet aangeboren hersenletsel (nah) graag op weg in het leven? Ben je een ervaren begeleider of juist net afgestudeerd en klaar om het vak verder te leren? Kom werken als ambulant begeleider, midden in het leven van Den Haag. Als ambulant begeleider lvb/nah (niveau 4), help je cliënten om zo goed mogelijk te participeren in de samenleving. Je biedt ondersteuning op maat er werkt samen met de cliënt aan doelen die in samenspraak met de WMO-consulent overeen zijn gekomen. Met jouw focus op eigen kracht en doorzettingsvermogen, werk je aan het verbeteren van de persoonlijke situaties en stabilisatie. De ondersteuning vindt plaats op diverse leefgebieden zoals wonen, werk, relaties en post en administratie. Hierbij krijg je veel verantwoordelijkheid en beheer je zelfstandig je eigen caseload. Verder ga je: - Samenwerken met naasten en samenwerkingspartners en zo je eigen netwerk opbouwen. - Werken met het Middin Kompas en volgens de Triple-C methodiek - Deelnemen aan overleggen. - Jouw eigen agenda en afspraken beheren. - Deelnemen of actieve bijdrage leveren aan o.a. de lotgenotengroep: DenkTank en Middin in de Wijk. - Actief deelnemen aan project of werkgroepen. - Kennis en ervaring delen met collega's. Je werkt in het gebiedsteam Den Haag van Middin. Het team bestaat uit ongeveer 34 mensen. We gaan uit van een brede basis, zodat je uitwisselbaar bent en het team flexibel blijft om alle zorgvragen zo goed mogelijk te kunnen beantwoorden. We werken in 4 sub teams. De voorzitter is een collega uit je team. Daarnaast leert iedereen, elke dag weer. Dat past bij ons team, bieden we ook alle ruimte voor en daar gaan we dus ook bij jou vanuit.
