Dit doe je voornamelijk in de thuissituatie van de cliënt of in een poliklinische setting. Jij beoordeelt en diagnosticeert je cliënten, regelt de inzet van zorg en draagt bij aan het behandelplan en de uitvoering ervan - waarbij jij de cliënt en zijn haar netwerk zo veel mogelijk betrekt en voorlicht. Daarnaast: - intervenieer je in crisissituaties; - neem je deel aan de multidisciplinaire overleggen en werk je samen met ketenpartners; - investeer je in je eigen ontwikkeling via de aangeboden cursussen en trainingen.
