Ambulant Jeugd en Gezinsbegeleider Organisatie: Ipse de Bruggen Locatie: 's-Gravenhage Opleidingsniveau HBO Uren per week 24 - 36 uur per week Solliciteren kan tot en met 03 oktober 2026 Wij zoeken een ambulant jeugd en gezinswerker voor ons integrale ambulante team Beter Thuis in Den Haag. Hier werken we samen met Youz, Jeugdformaat en Ipse de Bruggen aan herstel van het gewone leven voor kinderen, jongeren en hun gezin. Wie zijn wij Ipse de Bruggen is de grootste zorgorganisatie in Zuid-Holland voor mensen met een verstandelijke beperking. Wij zijn gespecialiseerd in complexe zorgvragen. Op 370 zorglocaties bieden wij met ruim 5.500 medewerkers en 2.200 vrijwilligers bijna 5.700 cliënten zorg en ondersteuning bij wonen, werken, leren en ontspannen. Onze missie is: zorgen dat cliënten een goed leven hebben en zich kunnen blijven ontwikkelen. Wij geloven dat er altijd meer mogelijk is. Hoe klein het stapje ook is. Het vernieuwende Integrale project Beter Thuis Zoetermeer en Den Haag is een gezamenlijk initiatief van Jeugdformaat, Ipse de Bruggen, Youz, Parnassia volwassen GGZ. Onze doelgroep zijn kinderen en jongeren en complexe gezinnen. Binnen Beter Thuis verbinden we onze deskundigheid en bundelen we onze krachten. We kijken welke problemen gezinnen hebben en hoe we hen daarbij samen het beste kunnen helpen. We verwijzen dus niet meer door, maar zijn er als Beter Thuis voor de jongere en zijn gezin en helpen hen samen weer het heft in eigen hand te nemen. Jouw kantoorlocatie wordt de Carel Reinierskade te Den Haag. Wij hebben een gang met vergader- en gesprekskamers, samen met het integrale 0-6 team in de wijk. Je werkplek is voornamelijk bij de gezinnen thuis. Jouw baan als Ambulant Jeugd en Gezinsbegeleider Wat ga je doen? - Je gaat samen met andere hulpverleners gezinnen op maat begeleiden en coördineert de zorg en ondersteuning van 'jouw'...
