Wij zoeken een energieke, proactieve collega die systemisch werkt en energie krijgt van het helpen van volwassenen en gezinnen. De rol van ambulant ondersteuner volwassenen is volop in ontwikkeling en samen met je collega's geef je hier vorm aan. Als ambulant ondersteuner volwassenen help je mensen van alle leeftijden met weer zelfstandiger meedoen in de maatschappij. Dat gaat over school, werk, opvoeding, ontwikkeling, financiën, wonen, sociaal contact. Hierbij is expertise en affiniteit met de doelgroep ouderen een pré. Samen met de inwoner en diens netwerk maak je een plan. Je helpt bij het plannen en regelen van ondersteuning. De aanpak is systeemgericht, preventief, tijdelijk en gericht op het versterken van zelfregie en het normale leven. Je doet dit in goede verbinding met de lokale voorzieningen die beschikbaar zijn. Waar nodig bied je zelf ondersteuning en evalueer je het bereikte resultaat, waar nodig zet je specialistische ondersteuning in. Naast de individuele ondersteuning signaleer en bundel je hulpvragen zodat er een collectief aanbod kan worden ontwikkeld/ingezet. Wat breng je mee naar Goirle? • Een afgeronde, relevante hbo opleiding • Flexibiliteit, creativiteit en leergierigheid • Je beschikt over relevante werkervaring in een soortgelijke functie met kennis van wet- en regelgeving • Je werkt systematisch, methodisch en vanuit een integrale benadering Functie-eisen: Uw kandidaat heeft minimaal een hbo werk- en denkniveau, zoals social work, sociaal maatschappelijke dienstverlening of pedagogiek.
