Logo GGZ Friesland 48 Ambulant Verpleegkundige - regioteam Franeker HBO 32 - 36 uur Locatie Franeker Sluitingsdatum: 4 oktober 2026 Geplaatst op: 15 september 2026 Als ambulant verpleegkundige bij het regioteam Franeker ondersteun je patiënten met ernstige psychiatrische aandoeningen, vaak in combinatie met complexe problematiek op verschillende levensgebieden. Je werkt vanuit een herstelgerichte visie en helpt patiënten om zoveel mogelijk regie over hun eigen leven te behouden of terug te krijgen. Je begeleidt en behandelt patiënten voornamelijk in hun eigen leefomgeving. Daardoor krijg je een goed beeld van hun dagelijkse situatie, bouw je aan een duurzame behandelrelatie en bied je zorg die aansluit bij wat de patiënt nodig heeft. Je bent de verbindende schakel binnen het netwerk rondom de patiënt en zorgt voor samenhang in de zorgverlening. Je signaleert veranderingen, beoordeelt risico's en schakelt tijdig wanneer de situatie daarom vraagt. Daarbij werk je nauw samen met collega's, naasten en ketenpartners om de zorg goed op elkaar af te stemmen en de continuïteit van zorg te waarborgen. Ook in crisissituaties weet jij snel te handelen en passende ondersteuning of vervolgzorg te organiseren. Zo draag je iedere dag bij aan herstelgerichte, persoonsgerichte en kwalitatief hoogwaardige zorg voor patiënten in de regio Franeker.
