deRotterdamseZorg - Ambulant Woonbegeleider | Schiedam Pameijer zoekt een Ambulant Woonbegeleider | Schiedam Vanuit Woont Schiedam Nieuwland zoeken we een Woonbegeleider (Wooncoach A) voor teamuitbreiding. Het is een uitdagende locatie. Iemand moet sterk in zijn schoenen staan en om kunnen gaan met verslavingsproblematiek en mensen met psychische klachten. Wij hebben zowel intramuraal als extramuraal klanten. Wat ga je doen? Je komt te werken op een uitdagende locatie in Schiedam. De bewoners zijn tussen de 20 en 70 jaar oud. De locatie bestaat uit 30 zelfstandige woningen en daarnaast wonen circa 50 ambulante cliënten in de wijk begeleid. Je werkt zowel ambulant als op de woongroep (twee keer per week). Je bent verantwoordelijk voor je eigen planning en geeft zelfstandig invulling aan je ambulante werkzaamheden, met ruimte om je voorkeuren aan te geven voor thuishavendiensten. Als wooncoach ben je persoonlijk begeleider van een aantal bewoners. De bewoners hebben voornamelijk psychische beperkingen, soms in combinatie met middelengebruik. We zoeken iemand die stevig in zijn of haar schoenen staat en duidelijk grenzen kan stellen. Samen met de bewoners stel je doelen op en werk je hier zelfstandig aan. Tegelijkertijd is het belangrijk dat je je houdt aan de werkafspraken, zodat er voor alle bewoners duidelijkheid is en er eenduidig wordt gewerkt. Zo doen we bijvoorbeeld samen met de bewoners boodschappen. Je werkt volgens een onregelmatig rooster met dag-, avond- en weekenddiensten. Als Woonbegeleider: - Stem je doelen af met de cliënt, verwerk je deze in het ondersteuningsplan, bewaak je de voortgang en rapporteer je hierover. - Ondersteun je de cliënt bij algemene dagelijkse levensverrichtingen (ADL). - Registreer je de werkzaamheden en verricht je administratieve werkzaamheden, die voortvloeien uit de functie. - Ondersteun je ook het (sociale) netwerk van de cliënt met advies en informatie.
